Patient N.T.T.T (1959, Ho Chi Minh City) have been suffering left displaced tibia and fibula fractures in which the case required surgical intervention to join the 2 broken parts together. Respect patient choice of therapy, City International Hospital’s medical staff successfully performed a major leg surgery without transfusion of allogeneic blood.

September 25th 2018, Patient N.T.T.T got accepted to City International Hospital after a traffic accident. After pre-operative assessment, the patient was diagnosed of third-tibia fractures as well as for open fractures of left leg, oblique with fragmented wedge, tibialis anterior muscle, broken ankle joint and soft tissue injured.

After Visual inspection and Imaging Tests, our medical team determined a fracture fixation would be the best treatment in other to avoid the incidence and risk factors of surgical delay. However; the patient and her family was asking a surgical procedure that is performed without a foreign blood transfusion because religiously he could not receive blood transfusion from any donor (not even from his family members). Respect patient choice of therapy, City International Hospital’s medical staff agreed and successfully performed a major surgery of tibia and fibula fractures without transfusion of allogeneic blood.

The One hour surgery was performed by a trained- team of doctors, starting with an incision carried out straight through the sub-cutaneous fat without developing skin flaps. Fibular fixation was accomplished with the use of a 7-hole with 7 vis 4.5, a 2.2 mm Kirschner-wire was inserted through the radial bone marrow cavity. During operation, the reduction of fracture and the location of titanium elastic nails were examined by C-arm.

After surgery, Dr. Tran Van Be, Orthopedic Department at City International Hospital, said this is a difficult case because the patient got Femur Fractures and the femur is a strong bone and is very hard to break. Without timely surgical intervention, the case might easily go out of hand and lead to dangerous complications such as: damaging nervous system, compartment syndrome, infection, limp amputation, swollen legs ... Fortunately, the surgery also went smoothly.

Morning September 16th 2018, the Board of Management and staff of the International Hospital City have visited the patients. In good recovery status, she positively shared: "Despite our family related-issue, my family and I had to refuse blood transfusion. Respecting the patient and their family on the choice of therapy, my lifesaving medical team has accepted the challenge which had been refused by many other practices I had been to before. Sharing with us: “Dr. Dao Thi My Van - Head of Anesthesia & ICU Department, International City Hospital who was also the anesthesiologist in charge: The amount of blood loss during fracture fixation can be reduced by using Garrote method. The patient only lost blood through open fractures during the period of transferring the patient from his hometown to Ho Chi Minh City; transfusion requirements might not be that big of the problem. However in femoral fracture, the femur is very vascular, and fractures can result in significant blood loss into the thigh, where refusal of blood transfusions will be a life and death issue in medical practice.  For this issue, we decide to use Cell Saver —a medical device that involves recovering blood lost during surgery and reintroducing it into a patient’s body.

Physical therapeutic training and follow-up visits are highly recommended for quick recovery and normal life activities.
