13 April 2019, one hour to Midnight, a 47-year-old woman Y.S (Phnompenh) presented to City International Hospital (CIH)’s accident & emergency department (A&E) with left lower quadrant pain, which had started 5 days earlier and was constant, dull, aching, and non-radiating.

There were no aggravating or alleviating factors. The pain was associated with low-grade fever and nausea. She reported no vomiting, no change in bowel habits, and no hematemesis, hematochezia, or melena. She did not have urinary urgency, frequency, or dysuria. She had no cardiac, respiratory, or neurologic symptoms.

Clinical history of the patient, In Phnompenh 5 days ago the patient was having epigastric abdominal pain accompanied by nausea and fatigue. At Khema International Clinic, she was admitted with an initial diagnosis of acute diverticulitis. She was started on antibiotics, hydration, and pain medications, and her abdominal pain gradually improved. , so her family members decided to travel to Vietnam’s City International Hospital seeking for help.

At City International Hospital, The patient underwent MR imaging and CT imaging upon admission were performed upon admission. The findings revealed which revealed severe gallstone pancreatitis with increased liver enzymes due to gallstones blockage inside the bile duct which resulted in a severe gallstone pancreatitis. Initially, the patient was given medications. After 2 days of hospitalization, the patient underwent another abdominal CT scan, Assoc. Prof. Dr. Nguyen Tan Cuong - Medical Director-Head Department-Senior surgeon (Deaprtmant of GI & Hepato-Biliary-Pancreatic (HPB) Surgery) has evaluated the liver enzymes status of Y.S patients was stable, around 11:30 a.m.16 April 2019, CIH's team of expert surgeons conducted a surgical removal using endoscopic cholecystectomy. Assoc. Prof. Dr. Nguyen Tan Cuong - CIH's Medical Director explained, acute pancreatitis is one of the most common diseases in the gastrointestinal tract. Gallstone pancreatitis recurs in 30% to 50% of people after an initial attack of pancreatitis. arly recognition of the severity of the attack, therefore, is of paramount importance, as therapy may be altered. Surgical removal of the gallbladder (cholecystectomy) is often recommended during the same admission in mild cases to prevent a recurrence. In elderly patients or those with serious medical problems, it may not be safe to remove the gallbladder. In this case, ERCP can be done to enlarge the bile duct opening. This allows stones from the gallbladder to pass, helping to prevent recurrence.

My family and I thought my loss of appetite was due to my suffering a right upper quadrant pain. However, the symptoms continued for weeks; I could not sit straight because of the pressure and pain just under my ribs, and extreme fatigue had been an issue for some time. Also, I experienced abdominal cramps and pain every time I tried to eat, usually within half an hour, sometimes after several hours. Upon arrival to City International Hospital, I had lost 10kg. I was so lucky to be saved by a  powerful team of top surgeons and so happy to get home, seeing the sunshine, birds and flowers in my garden was rejuvenating. After a few days of continuing care, very quickly I began to recover. Generally  my family and friends have been very kind and supportive. Having the herosurgeon, Assoc. Prof. Dr. Nguyen Tan Cuong explained, reason for surgical removal for this particular case is mandatory because due to large calcium stones that were blocking the main pancreatic duct. Another procedure which may help in some people is to 'stretch' wide a narrowed pancreatic duct to allow better drainage of pancreatic enzymes. Surgery may also be needed if a complication develops. For example, if a blocked bile duct or pseudo-cyst develops. Severe pancreatitis can lead to potentially life-threatening complications, including damage to the heart, lungs, and kidneys. Surgeons may remove your gallbladder (called a cholecystectomy) if gallstones (or other types of gallbladder disease) are causing problems. Techniques include laparoscopic (‘keyhole’) cholecystectomy or open surgery. The gallbladder is not a vital organ, so your body can cope quite well without it.

The patient was discharge after a few days.
